Very often, when you solve a problem by asking “why” only one time, you will see only what’s on the surface. Asking this question five times will get you to the root. That’s why it’s so powerful.

    Cooperation with the team will help you to get better answers.

  2. Be focused on finding the solution, and preventing the problem in the future.
    Don’t blame others for their mistakes.
